Service Line and Riser Certification D.O.T. Form

Service Line Installation Must Comply With The Following Standards:

1. Material must comply with ASTM D2513
2. 10 or 12 gauge solid copper yellow thermoplastic tracer wire
3. Service installed to a minimum depth of 30 inches
4. Both ends of pipe sealed to prevent water and dirt from entering pipe
5. Service line is not run under or through any structure
6. Riser is properly supported and must be installed 1' above final grade
7. Meter is protected from vehicular damage
8. Meter set is not closer than 3 feet to any ignition source
9. Meter is properly supported

* By signing this form, the installer attests that they installed or repaired the service line and/or meter set in accordance with applicable codes
and standards. The signer further attests that:

1. He/she is Operator Qualified in accordance with the requirements in 49 CFR 192, Subpart N and has been evaluated by a Waterville Gas
   approved third party qualifier and has been deemed qualified to perform the covered tasks associated with service line installation. Must requalify every 3 years.
2. His/her employer administers a Drug & Alcohol Testing Program that meets 49 CFR 199, including random testing.
